At United American Health Association (“UAHA”), keeping member information secure is a top priority. By interacting with UAHA through our websites, mobile applications, products, and services, you consent to the use of information collected or submitted as described in this privacy policy. Your Personal Information
The personal information members provide UAHA is collected solely for the purpose of providing members and their dependents (if applicable) with the UAHA services and benefits. This information is protected by UAHA by maintaining physical, electronic, and procedural safeguards.
Information We Collect
Personal information we collect about members comes from one or more of the following sources: web application, your employer (if applicable), membership association (if applicable), or partnered groups. Personal information collected includes and is not limited to: name, mailing address, telephone numbers, e-mail address, social security number (when necessary), date of birth, gender, geolocation, mobile device information, dependent name, dependent gender, dependent date of birth, dependent social security number (when necessary), dependent relationship, bank account information, or credit card information.
We may disclose any or all this information based on business need only, to our partners, service providers, third party service providers, provider networks and other suppliers of products and services to UAHA members.
UAHA will maintain member information even after membership is terminated for any reason. This information will be protected in the same manner as that of an active member. A member may request in writing at any time after discontinuing the membership to remove the information from our file, subject to business record keeping requirements for administration or reporting purposes.
Sharing Information with UAHA and Service Providers
UAHA will share member information with its partners, related companies and service providers to the extent it is necessary to provide and manage benefits to the member or their dependents. Unless otherwise permitted by law UAHA will not share with these parties any information it collects that is not needed by these parties to provide the benefits to the member or their dependents. These organizations are not authorized to use member information for any purpose other than for UAHA business.
Sharing information with Third Parties or Service Providers not affiliated with UAHA.
UAHA will not share, sell or otherwise disclose to any third parties not affiliated with UAHA member information collected by UAHA. This includes other service providers and marketing organizations not directly related to providing UAHA member services.

UAHA Commitment to Members
Keeping UAHA member information secure and using it only as needed for business reasons to provide services to UAHA members is a top priority. UAHA will safeguard, according to strict standards of security and confidentiality, any information provided by our members. UAHA will permit only authorized employees who are trained in the proper handling of member information to have access to that information. Employees who violate UAHA Privacy Policies will be subject to normal disciplinary processes.
UAHA will not reveal member information to any external organizations (other than partners and service providers as mentioned), unless the member specifically authorizes UAHA to do so or as required by law. UAHA will always maintain control over the confidentiality of our member information. Whenever we hire other organizations to provide support or services to UAHA, and will require them to conform to UAHA privacy standards and allow UAHA to audit them for compliance. UAHA will attempt to keep member files complete and up to date and accurate. A member can access their member portals to correct or update any information at any time. UAHA will not make available information collected in connection with or in anticipation of a claim or legal proceeding.
